A worker at a Southern California frozen food plant died last weekend after becoming caught in machinery at the facility, according to local reports.

Law enforcement officials told KABC-TV that the worker, 19, was cleaning an industrial food processor at the Tina’s Burritos plant in Vernon, California, the night of July 13 when it unexpectedly activated. Other workers at the plant, according to the report, were unable to turn the machine off.

The victim, whose name was not released, died at the scene.

California workplace safety regulators were expected to begin an investigation of the incident early last week.
